REGISTERED NURSE (Home Care Floater Team)

Job Responsibilities:

  • Perform biopsychosocial and environmental assessments to identify strengths and needs of clients, families and/or caregivers.
  • Plan, develop and evaluate person-centred nursing care plans with considerations of client’s needs and preference. Communicate care plans and address the concerns of patient and family.
  • Implement and evaluate care interventions(s) e.g. wound care, medication management, continence care; ensuring at all times:
  • Practice safety and infection control precautionary measures.
  • Manage patient workload through timely planning and scheduling based on the client’s needs. Utilise workforce optimiser to actualise the planned visits.
  • Build therapeutic relationships and empower the clients, families and/or caregivers for self-management through health promotion, caregivers training and motivational techniques.
  • Coordinate with multidisciplinary team for care integration and transition
  • Initiate emergency protocols when necessary.
  • Discuss/escalate to Supervisor and/or Care team on any problems encountered in the patient’s condition and/or in the handling of patients and/or caregivers/family members.
  • Develop discharge plan and refer patients to other service providers to transit care.
  • Supervise the work of the Enrolled Nurses in his/her zone, as well as nursing students on clinical attachment to achieve their learning objectives.
  • Perform visits during Emergency Nursing Services for normal and extended hours.
  • Maintain timely and accurate documentation of patients’ conditions and care given.
  • Collect correct fees by checking the subsidized status of patients, issue receipts and submit the collection in a timely and accurate manner.
  • Facilitate the application of healthcare financial schemes such as Senior Mobility Funds when appropriate.
  • Exercise appropriate use of resources and supplies.
  • Comply with guidelines and policies to ensure client confidentiality and personal data protection.
  • Keep abreast of new technology and nursing knowledge through reflection on practice and continuous professional development.
  • Support quality audit to maintain and improve standards of care.
  • Assist the (Senior) Nurse Manager and/or Nurse Clinician to improve, facilitate and implement changes within the home nursing work processes.
  • Assist in committee/workgroup activities as directed by (Senior) Nurse Manager and/or Nurse Clinician.

Apart from the above description, the Floater will also have the following job duties:

Centre Based Services including HNF and Outsource Centre(s)

  1. Perform assessment of clients for suitability of admissions.
  2. Perform assessment, care planning and outcome evaluation of the client on admission, every 6 monthly and whenever the patient’s condition changes.
  3. Plan, implement and evaluate nursing procedures / management.
  4. Ensure safe administration of medications, and timely and accurate documentation including supervision of nursing aide during the administration process.
  5. Report any medication error or adverse drug event to Centre Manager and raise report, if required.
  6. Supervise the care staff to maintain a safe environment and practices as well as adhere to infection control practices.
  7. Educate and train patients and caregivers with the required skills and knowledge to achieve independence and self-reliance as much as possible.
  8. Ensure clinical related documentation is updated in a timely manner.
  9. Develop discharge plan and refer patient to another service provider to transfer care if necessary.
  10. Escalate care to the Nurse Clinician in an appropriate and timely manner.
  11. Perform assessment and render first aid measures when there is injury incurred.
  12. Initiate and lead resuscitation or first aid procedures for various clinical emergencies.
  13. Assist Centre Manager in reviewing of client’s clinical condition and suitability for programme(s) in the centre such as outings and social activities.
  14. Act as a resource for clinical related issues to the Centre Manager.

Caregiver Training – workshops, volunteers, in-home

Home Nursing Procedures -Swab Activation

  1. Provide Swab and Serology Tests for patient upon activation.
  2. Ensure proper labelling of the specimen and identification of the client.
  3. Identify contraindication for swabbing.
  4. Perform swab and/or blood-taking procedure.
  5. Drop off the specimen collected in the assigned laboratory.

Coverage of home care services as assigned, including cross-coverage of teams

Other Outsource service(s) e.g. home bathing service as assigned.

Job Requirements:

  • Registered Nurse with the Singapore Nursing Board
  • Minimum 3 years’ experience working in acute hospital setting and/or community hospital setting
  • Possess Diploma in Nursing
  • Possess good communication skills to establish therapeutic relationship that fosters optimal quality patient care
  • Able to make clinical judgments regarding patient care with minimal guidance
  • Able to work on the following hours (5 day work-week):
    • Monday to Friday: 12.30pm to 9pm
    • Saturday: 8am to 5.30pm
