Main Responsibilities

• Supports planning and implementation of MSW practice protocols and programmes
• Provide psychosocial assessment, intervention and case management to clients and their next-of-kin
• Provide information, care coordination and referral services
• Engage with community partners such as community clubs, schools and childcare centres to develop and implement new community development initiatives
• Network with community stakeholders such as Agency for Integrated Care, Social Service Organisations and Restructured and Community hospitals for effective care integration
• Plan and implement programme and activities for Maintenance daycare, Dementia daycare and Enhanced Dementia daycare clients
• Ensure smooth execution of the Caregiver Training Programme including administering the curriculum and training schedules

Job Requirements

• Degree in Social Work
• Registered with Singapore Association of Social Work
• At least 5 years of social work experience in healthcare industry
