Registered Nurse

Job Responsibilities

Reporting to the (Senior) Nurse Manager and/or Nurse Clinician, the nurse will hold the following  job responsibilities:

  • Provide nursing care in the home environment. E.g. Wound care, stoma care, continence management, diabetic education while ensuring the patient’s dignity, safety and their confidentiality is maintained at all times. 
  • Provide coordination and care management for assigned patients by: 

Assessment

Care Planning

Implementation of care

Outcome evaluation

Make referral to other members of the healthcare professional team (doctor, social welfare executive, etc.)

Maintain timely and accurate documentation of patients’ condition and care given.

  • Review and discuss with (Senior) Nurse Manager and/or Nurse Clinician on any problems encountered in patient’s condition and/or in the handling of patients and/or caregivers/family members.
  • Provide caregiver training programme and health education to patients/caregivers and evaluate the teaching programmes to meet patients and caregivers needs and expectations
  • Keep abreast of new technology and nursing knowledge through continuous education and familiarization of new trend/management and equipment usage.
  • Collect correct fees by checking the subsidized status of patients, and submit e.g. Receipts to patients on the spot / Cash collected to office on next working day
  • Orientate and coach new nurses and serve as liaison, resource person and preceptor to nurses and trainees.
  • Supervise the work of the Enrolled Nurses in his/her zone.
  • To practice and be able to teach Standard Precaution protocols to junior nurses.
  • Exercise appropriate use of resources and supplies.
  • Able to perform visits during Emergency Nursing Services hours.
  • Assist the (Senior) Nurse Manager and/or Nurse Clinician to improve, facilitate and implement changes within the home nursing work processes.
  • Assist the (Senior) Nurse Manager and/or Nurse Clinician to handle complaints and initiate service recovery.
  • Assist the (Senior) Nurse Manager and/or Nurse Clinician to audit/appraise the performance of new/junior nurses.
  • Participate in committee/workgroup activities as directed by (Senior) Nurse Manager and/or Nurse Clinician.
  • Assist to supervise the junior nurses, as and when required. 
  • Perform other related duties as and when directed by said reporting officer(s).

Job Qualifications:

– Registered Nurse with the Singapore Nursing Board

– Minimum 5 years’ experience working in restructured hospital setting. Experience in Home Nursing preferred

– Possess Degree and/or Advanced Diploma in related fields
