This Data Protection Policy supplements but does not supersede nor replace any other consents you may have previously provided to HNF in respect of your Personal Data, and your consents herein are additional to any rights which we may have at law to collect, use or disclose your Personal Data.

HNF may from time to time update this Data Protection Policy to ensure that this Data Protection Policy is consistent with our future developments, industry trends and/or any changes in legal or regulatory requirements. 1.      Personal Data

In this Data Protection Policy, “Personal Data” refers to any data, whether true or not, about an individual who can be identified (a) from that data; or (b) from that data and other information to which we have or are likely to have access, including data in our records as may be updated from time to time. 

Examples of such Personal Data you may provide to us include (depending on the nature of your interaction with us) your name, NRIC, passport or other identification number, telephone number(s), mailing address, email address and any other information relating to any individuals which you have provided us in any forms you may have submitted to us, or via other forms of interaction with you.

2.     Collection of Personal Data

Generally, we collect Personal Data in the following ways:

  • when you submit any form, including but not limited to application and registration forms;
  • when you enter into any agreement or provide other documentation or information in respect of your interactions with us, or when you use or request for or are referred to our services;
  • when you are subject to or participate in a medical examination or when you interact with our staff and affiliated / locum general practitioners, for example, via telephone calls (which may be recorded), letters, face-to-face meetings and emails;
  • when you use our electronic services, or interact with us via our websites or use services on our websites;
  • when you request that we contact you or request that you be included in an email or other mailing list;
  • when you respond to our initiatives or to any request for additional Personal Data;
  • when you submit an employment application or when you provide documents or information including your resume and/or CVs in connection with any appointment as a social worker, nurse, medical care provider or any other position;
  • when your image is captured by us via CCTV cameras while you are within our premises, or via photographs or videos taken by us or our representatives when you attend events organised or hosted by us;
  • when you are contacted by, and respond to, our staff, nurses, doctors, agents, representatives and other service providers;
  • when you make payment or provide details to facilitate payment, or secure or administer the application of funding / benefits / subsidies;
  • when we seek information about you and receive your Personal Data in connection with your relationship with us, including for our services or job applications, for example, from healthcare partners, public agencies, your ex-employer, referral intermediaries and the relevant authorities; and/or
  • when you submit your Personal Data to us for any other
  • when you browse our website, you generally do so anonymously and we do not, at our website, automatically collect Personal Data unless you provide such
  • if you provide us with any Personal Data relating to a third party (e.g. information of your spouse, children, parents, and/or employees), by submitting such information to us, you represent to us that you have obtained the consent of the third party to provide us with their Personal Data for the respective.

You should ensure that all Personal Data submitted to us is complete, accurate, true and Failure on your part to do so may result in our inability to provide you with the services you have requested.

3.      Purposes for the Collection, Use and Disclosure of Your Personal Data

Generally, HNF collects, uses and discloses your Personal Data for the following purposes:

  • responding to your queries, feedback, complaints and requests;
  • verifying your identity;
  • managing the administrative and business operations of HNF and complying with our internal policies and procedures;
  • requesting feedback or participation in surveys, as well as conducting research and/or analysis for statistical, profiling or other purposes for us to understand, review, develop and improve the quality of our services;
  • matching any Personal Data held which relates to you for any of the purposes listed herein;
  • preventing, detecting and investigating crime and analysing and managing commercial risks and conducting investigations relating to disputes, billing or fraud;
  • managing the safety and security of our premises and services (including but not limited to carrying out CCTV surveillance and conducting security clearances);
  • monitoring or recording phone calls and customer-facing interactions for quality assurance, employee training and performance evaluation and identity verification purposes;
  • in connection with any claims, actions or proceedings (including but not limited to drafting and reviewing documents, transaction documentation, obtaining legal advice, and facilitating dispute resolution), and/or protecting and enforcing our contractual and legal rights and obligations; and/or
  • meeting or complying with any applicable rules, laws, regulations, codes of practice or guidelines issued by any legal or regulatory bodies which are binding on HNF (including but not limited to responding to regulatory complaints, disclosing to regulatory bodies and conducting audit checks, due diligence and investigations).

In addition, HNF collects, uses and discloses your Personal Data for the following purposes depending on the nature of our relationship:

  • If you are an existing or prospective patient of HNF:
  • conducting means testing, processing financial assistance, conducting intervention (including early intervention), and arranging for special programmes;
  • onboarding information on the relevant individuals necessary to establish patient records and to commence treatment and care of the patient;
  • managing your relationship with us, including providing medical treatment, services and advice, the management of your house visits, registration, counselling, advising you of alternative treatment options, sending notifications to you, communicating patient care issues and securing instructions on treatment choices;
  • contacting your next-of-kin / caregivers / representatives / legal guardians for purposes of arranging home visits, providing medical updates, and seeking consent from them in emergency / incapacity situations;
  • ensuring appropriate delivery of core patient care services including delivering results of tests and other medical updates and facilitating the provision of healthcare services equipment;
  • ensuring proper and complete diagnosis and appropriate treatment including and without limitation to identifying health / treatment risks (e.g. collecting, identifying and communicating vulnerabilities, conditions, allergies, potentially adverse reactions ) and monitoring appropriateness of medication usage, specimen testing and reporting recording patient infection data;
  • prescribing and dispensing appropriate medication;
  • ensuring a patient’s health and safety (including, for example, advising you of a product or drug recall);
  • administering compulsory / legally mandatory medical procedures (e.g. vaccinations);
  • internal auditing, managing medical records, including facilitating patient merge processing and answering requests for medical records (including compilation of medical records for quick reference for medical purposes), and producing medical reports and associated administrative documents;
  • co-ordinating healthcare services provided by other healthcare providers;
  • managing referrals / collaboration with / transfer of patients to and from other institutions, healthcare professionals, caregivers, persons, organisations for procedures, additional support on treatment, specialist assistance, the procurement or provision of follow up care;
  • conducting medical reviews to better address your medical risks and improve patient care;
  • providing social support services (including but not limited to delivering food items and personal hygiene products); and/or
  • working with funeral directors, casket companies, morticians, or other like persons as may be necessary to discharge duties with respect to a deceased individual.
  • If you are a next-of-kin, family member, caregiver, representative or legal guardian of one of our patients:
  • processing your application for our services on behalf of the patient;
  • conducting means testing, processing financial assistance, conducting intervention (including early intervention), and arranging for special programmes;
  • facilitating the provision of our services (including but not limited to providing home-nursing care, home medical care and social welfare services);
  • developing a care plan that best matches the patient’s requirements with his home environment (including but not limited to providing you with training to equip you procedurally and emotionally to care for the patient);
  • obtaining written consent for nursing procedures; and/or
  • contacting you in the event of an emergency.
  • If you are a donor, sponsor, volunteer, befriender or a guest attending our corporate event:
  • soliciting donations, tracking sponsorship, processing tax deductions for eligible donations by donors and filing mandatory financial reports with police licensing unit for donations received by our institutions;
  • administering volunteer programmes, organising fundraising events, projects, excursions and outreach initiatives (including but not limited to arranging and planning logistics, verifying your identify and recording attendance);
  • managing our relationship with you, including sending letters of gratitude and collaterals such as donation forms;
  • featuring pictures or videos of you (in accordance with the law) in collaterals or mass media for publicity purposes; and/or
  • creating and maintaining your profile in our database.
  • If you are an affiliated or locum general practitioner providing medical and healthcare services for and on behalf of HNF:
  • administrative and support processes relating to your providing of medical and healthcare services, including logistics management, processing expense claims and scheduling your appointments and creating and maintaining your profile in our database;
  • facilitating the communication between you and the patient and the patient’s next-of-kin / caregivers / representatives / legal guardians; and/or
  • managing our relationship with you and ensuring medical records are kept up- to-date.
  • If you are a visitor to our premises, facilities management (including but not limited to maintaining the security of our premises and recording entries and exits.
  • If you are an employee, officer or owner of an external service provider or vendor outsourced or prospected by HNF:
  • assessing your suitability as an external service provider or vendor for HNF;
  • managing project tenders and quotations, processing orders or managing the supply of goods and services;
  • processing and payment of vendor invoices and bills;
  • managing business operations; and/or
  • facilities management (including but not limited to maintaining the security of our premises and recording entries and exits).
  • If you are a member of our Board of Management:
  • complying with any applicable laws, regulations, rules, codes of practice or guidelines or to assist in law enforcement and investigations by relevant authorities (including but not limited to disclosures to regulatory bodies or conducting due diligence);
  • administrative matters, including submission to and/or registration with the relevant regulatory bodies and registries;
  • inclusion in reports, statements and other communications or collaterals which may be distributed to staff of HNF and/or members of the public; and/or
  • planning and organising corporate events and registration for conferences and training courses.
  • If you submit an application to us as a candidate for employment or representative position:
  • conducting interviews;
  • processing your application (including but not limited to pre-recruitment checks involving your qualifications and facilitating interviews);
  • obtaining employee references to conduct background screening;
  • assessing your suitability for the position applied for;
  • facilities management (including but not limited to maintaining the security of our premises and recording entries and exits); and/or
  • onboarding successful applicants.

Furthermore, where permitted under the Act, HNF may also collect, use and disclose your Personal Data for the following “HNF Additional Purposes”:

  • communicating to you advertisements involving details of our services, special offers and promotions, either to you generally, or which we have identified may be of interest to you; and/or
  • conducting market research and surveys to enable us to understand and determine preferences and demographics for us to offer you services as well as special offers and marketing programmes which may be relevant to your preferences and
  • In particular, HNF may collect, use and/or disclose your NRIC number, birth certificate number, passport number, work permit number or foreign identification number (or copies of the same) if required under the law (or an exception under the PDPA applies), or if it is necessary to accurately establish or verify your identity to a high degree of fidelity, including for the purposes set forth in clauses [3.2(a)(i), (ii), (v), (vii), (ix) to (xii) and 3(b)(i)] of this Data Protection Policy.
  • If you have provided your Singapore telephone number(s) and have indicated that you consent to receiving marketing or promotional information via your Singapore telephone number(s), then from time to time, HNF may contact you using such Singapore telephone number(s) (including via voice calls, text, fax or other means) with information about our
  • In relation to particular services or in your interactions with us, we may also have specifically notified you of other purposes for which we collect, use or disclose your Personal If so, we will collect, use and disclose your Personal Data for these additional purposes as well, unless we have specifically notified you otherwise.

4.    Disclosure of Personal Data

HNF will take reasonable steps to protect your Personal Data against unauthorised Subject to the provisions of any applicable law, your Personal Data may be disclosed, for the purposes listed above (where applicable), to the following entities or parties, whether they are located overseas or in Singapore:

  • other healthcare partners, healthcare institutions and parties providing referrals of patients to HNF;
  • our affiliated panel of general practitioners;
  • caretakers of patients, including next-of-kin, caregivers, representatives and legal guardians;
  • our fundraising and charity partners and trusted collaborators who provide community resources;
  • agents, contractors or third party service providers who provide operational services to HNF, such as laboratory and medical research services, courier services, telecommunications, information technology, payment, printing, billing, technical services, training, market research, security or other services to HNF;
  • external banks and their respective service providers;
  • our professional advisers such as consultants, auditors and lawyers;
  • relevant government ministries, regulators and professional regulatory bodies, registries statutory boards or authorities or law enforcement agencies to comply with any laws, rules, guidelines and regulations or schemes imposed by any governmental authority; and/or
  • any other party to whom you authorise us to disclose your Personal Data

If you withdraw your consent to any or all use of your Personal Data, depending on the nature of your request, HNF may not be in a position to continue to provide its services to you, administer any contractual relationship in place, which in turn may also result in the termination of any agreements with HNF, and your being in breach of your contractual obligations or HNF’s legal rights and remedies in such event are expressly reserved.

6.  Retention of Personal Data

HNF shall cease to retain any documents containing personal data as soon as if it is reasonable to assume that the purpose for which that data was collected is no longer being served or that retention is no longer needed for legal or operations purposes.
